Family Assistance Forms

Register a Family
To apply for family assistance, you must:
- Complete the application below. (Review instructions and guidelines here.)
- Have the parent/guardian sign the Media Release Form, including a jpg or png photo of the patient before or after treatment.
Please click the button below to complete the Family Assistance Application. You will need to ask parents/guardians to sign on your device, since this site is restricted to approved social workers.
All information is confidential and is intended for IronMatt’s use only.
Media Release
A media release is required. We cannot process an application without it. Please have the parent/guardian read and sign the release when they sign the application above.
Monthly Bill Submissions
Each month, you may submit up to three bills for a family that has been approved for assistance, using our bill submission form. (If the family has more than three bills, please reach out to the IronMatt team through the message box above.)
Please note that submissions are due by the 20th of each month, and bills are paid on the first of each month.
Instructions
EVERY FAMILY ASSISTANCE APPLICATION MUST:
- Be completely filled out, with signatures from both the social worker and a parent. Access the form here.
- Be accompanied by the Media Release, signed by the patient’s parent/guardian.
EVERY BILL SUBMISSION FORM MUST:
- Include a PDF/JPG image of the coupon page of the bill(s) – making sure it is legible and has the following information:
- Name to make check payable
- Address where to mail the payment
- Account number
- Be submitted by the patient’s hospital social worker by the 20th of the month.
Please note that:
- WE DO NOT PAY ONLINE BILLS.
- WE DO NOT PAY MEDICAL BILLS.
- WE PAY TO THIRD PARTIES ONLY. NO PAYMENTS TO THE FAMILY.
Some examples of bills we will consider:
- Rent/Mortgage
- Car payments
- Utility bills
- Lodging at long-term stay hotels for treatment
- Funeral expenses
Guidelines
Applicants must be 21 or younger and undergoing treatment for a brain tumor.
- Applications must be completed in full and submitted by a hospital social worker.
- Applications, bills, and photos must be received by the 20th of the month. We review applications once a month, and payments are sent out on the 1st of the following month.
- The Foundation only makes payments directly to third-party service providers.
- All third-party bills must accompany an application. Make sure the coupon page is uploaded to the application.
- The Foundation determines financial assistance based on a review of the facts and circumstances surrounding the request.
IMPORTANT: The media release is a requirement. Please make sure that it’s signed and returned with a jpeg photo of the child. The application will not be processed without these items.
